I acknowledge and agree by checking this box, in lieu of a physical signature, that Make It Happen
Sports may charge a non-refundable service fee of $25 if I cancel my registration less than 3 days
prior to the start of any event. Furthermore, I understand that my participation in Make it Happen
Sports involves risk and dangers of serious and permanent bodily injury and death. I, or my
parent/guardian, if I am a minor, hereby release, hold harmless, discharge and agree not to sue Make
It Happen Sports, Luke Fashaw, its directors, officers, employees, coaches, officials, volunteers,
agents, sponsors, advertisers, owners/leasers of premises for all liability from my participation in
these and any other related travel, lodging, social/recreational activities. Make It Happen Sports
will give you a credit towards a future camp minus the $25.00 service fee. This credit will be valid
for one year from the date of the canceled event. No refunds or credits will be given for any
cancellations on or after the first day of the event. All cancellation requests must be in writing
with no exceptions.  I also understand Make It Happen Sports retains the right to use for publicity
and advertising, photographs and video taken of the participants.
Player registration is in full swing as we gear up for the Summer sessions. We are currently filling teams for boys 10U,
11U,12U, 14U, 15/16U, and girls 7/8th grade,  and high school. If interested in participating please call/email our offices or
fill the player registration form, and fax/mail it back.
